You asked for clarification on a part of the book…”never overlook failure to carry one out.” Translation – Don’t ignore insubordination. I have found several ways to effectively address this, but his advice was simply stating that avoiding insubordination or a failure to follow an order by choice is poor leadership and ultimately will encourage your troops to continue on with bad habits. Hope this helps and that I’m not the 20 millionth person to point this out. Love your work. Thank you.

The author wrote, “Never overlook failure to carry one out.”
I believe that he meant, “Do not overlook an instance where your team failed to carry out an order.”
